Sample Lease
Rent Payments
Rent is due on the 1st of every month. It is late effective 5:01pm
on the 5th of every month regardless of what day the 5th falls on. After
5:00pm on the 5th, rent must be paid in cash or certified funds and
include all late fees.
Office hours are Monday - Thursday, 8:30am to 5:00pm. Our office is closed
on weekends. The outside door to the building is locked after 5:00pm
and locked on weekends.
In cases of inclement weather, call the office first to make sure we
are open.

Automated Clearing House (ACH) Request Form
To have your rental payment automatically withdrawn from a checking account, print out the Automated Clearing House (ACH) Request Form and return to the Select office.
Work Requests
All service requests must be submitted in writing unless it is an emergency.
You can print one out from the above link and return to our office by
mail or fax to us at 593-0529.
If you have an emergency, no heat, no electricity or running water, during business hours please contact the office. If your emergency is after business hours call the cell phone at 719-651-6791 and leave a message if no one answers. You MUST leave your number for us to return your call.
Requests for repairs must be directed to Select. If you contact the
vendor directly, you will be responsible for payment to that vendor.
Condition Report
This report is used to itemize any and all deficiencies in the house.
It is assumed that each house is clean and in good condition. If otherwise,
please note. Be specific in all remarks. This report is the same report
that Select Properties uses at move-out and periodic inspections.
You will need to print out a copy of this report and return to Select's
office within 5 days of your move in.
Moving Out
Notice to Vacate
Written notice must be given on the first of any given month to Select
to show your intent to vacate the property located. There is a move-out
checklist and approved carpet vendor list you will receive once
this notice is received by Select. All keys and garage door opener
remotes (if applicable) must be returned to Select's office by close
of business on the last day of the lease. You must also provide Select
with a forwarding address and a receipt for carpet cleaning. As per
the lease agreement, Select has 60 days from the lease expiration
or return of keys and garage door opener remotes, whichever is later,
to return your security deposit.
Carpet Vendors
The following are the companies that we have worked with and who
have been approved as per your lease agreement, paragraph #11. You
must use a vendor from this list and provide a copy of the invoice
to our office. Please be advised that if one of the below vendors
is not used, as per the lease agreement, you may be subject to additional
carpet cleaning costs. These vendors all carry the proper insurance
and guarantee their work.

****Pet odors are a special problem and require expertise. Should
you have this kind of problem, please contact management for additional
instructions.
Move-out Checklist
This checklist should be used to assist you in your move-out procedure.
It is based on the same list the property manager will go through
when doing their move-out inspection. You can view and print a move-out
checklist from the above link or pick one up at the office during
office hours.
Rental Verifications
For rental verifications for the purpose of obtaining a mortgage there
will be a $25 charge per request for us to fill out the lender rental
verification form. Our office policy is that if there is an outstanding
balance on any account, the request will be denied and forwarded as
such to the lender.
